BH (Psychiatric) Medical Professional: The Behavioral Health Medical Professional (M, DO, DNP or NP) works collaboratively with members of an integrated healthcare team that provide an array of integrated healthcare care services which include working with mental health, opiate treatment, homeless populations, crisis and general substance abuse populations.
Certified Medical Assistant: Provides medical and office support services to Behavioral Health Medical Professional (BHMP) and multidisciplinary team within CODAC clinical settings.
Crisis Advocate: The Crisis Advocate I is responsible for providing and coordinating crisis intervention services during office hours and after hours. Position provides hospital advocacy, crisis intervention, and follow up services, information and referrals for anyone seeking sexual assault/abuse support through The Center’s crisis intervention programs: Sexual Assault Resource Service (SARS), 24 hour/7 day bilingual Crisis Line, Accessing Safety (TTY/TTD/Text) Line and Walk-In Services, as applicable.
Dedicated Recovery Coach: CODACs Dedicated Recovery Coach work as part of a multidisciplinary team and ensures the integration of medical, behavioral health and wellness care for members.
Outreach Engagement Specialist (MAT): Outreach Engagement Specialist is responsible for engaging individuals who have been identified by hospitals, emergency personnel, families, faith community and other social service agencies as having a substance use disorder, specifically those with Opioid Use Disorder (OUD), into treatment services.
Therapist (AZ Licensed): Responsible for providing a variety of outpatient therapy services to CODAC members and their families/families choice living with mental and physical illness and/or addiction disorders to aid in their integrative health, recovery and wellness recovery using an integrative model.
